Health Requirements for Incoming Students

As a first-year or transfer student, you’re required to submit the following forms by July 1 in order to pre-register for classes, access your dorm room, and participate in any EphVentures program during First Days Orientation.

All forms and instructions must be submitted electronically via the Williams College Health Services Patient Portal. You’ll have access to this portal once you receive your Williams email and active it. If you’re having trouble logging in, see Ephs Patient Portal FAQ.

Your medical information is strictly confidential and will not be released except at your request.

Medical History
All first-year and transfer students are required to submit a health history, and you’ll need to have a physical exam within 12 months of July 1 to complete this form. If you can’t schedule an appointment for your physical prior to this date, please submit all other forms and submit your physical form as soon as possible prior to your arrival. If you plan to play intercollegiate sports you must submit a physical within the 6-month period preceding their sport season: August 30 for Fall sports, November 1 for Winter and Spring sports.  

Immunizations
Williams College complies with Massachusetts law requiring all entering students to present evidence of immunity to certain diseases. Required  vaccines are in your Patient Portal. If your religious beliefs conflict with this requirement, you must submit a written statement requesting exemption each school year to [email protected]

If you have a food allergy, please complete this Food Allergy Advisory Form which will enable Dining Services and Health Services better understand how we can create a safe environment for you in our Williams community.

Optional: Release of Medical Information
Should you wish to request a copy of your student health record for grad school or employment, just complete and return this form by email at Health​@williams​.edu or via fax at 413.597.2982. (Please allow up to two weeks for processing your request.)
