I. MEDICAL HISTORY:
All entering students are required to submit a health history. Medical information is strictly confidential and will not be released except upon student request or if needed to provide care for the student.
Williams College complies with Massachusetts’ law requiring all entering students to present evidence of immunity to certain diseases before they may register for class. The required and recommended vaccines are on the Health Form. Students whose religious beliefs conflict must submit a written statement requesting exemption.
All medical history and immunization information needs to be completed and received by JULY 1st in order to pre-register for classes, access your dorm room and participate in any EphVentures program during First Days Orientation.
III. PHYSICAL EXAMINATION:
A physical exam within the past year is required.
Students planning to participate in any Williams College sports program including intercollegiate, intramural and club sports must have a physical exam within the past six months. You will receive separate information from the Athletics Department.
The required health forms are accessible and should be submitted through the Patient Portal. Detailed instructions, as well as .pdf files of the required physician forms, are also available at health.williams.edu/forms/. Please note that there are electronic forms that can only be completed on the Patient Portal and all forms should be uploaded to Health and Wellness Services using this secure method of submission.
